You can download application forms by clicking on the download link. These forms can be downloaded in two formats, RTF and PDF. RTF format can be opened by any Microsoft Word application. If you want to download the PDF format, you must have Adobe Acrobat Reader installed in your computer in order to open the PDF file. FORM A To be used for complaints to one of the Financial Institution's Branch or to the Head office.
FORM B To be used for complaints to the Ombudsman.
Note: If you want to make a
complaint, simply download the application form, fill it and send
it to the Ombudsman's Office by email fosril@sltnet.lk
or by post to: However, before you complain to the Ombudsman
make sure you have followed the Rules.
For example, you must have first complained to the financial institution
concerned. It is only if they do not attend to your complaint
